Detroit Wayne Integrated Health Network and Community Living Services have partnered in the development of a Person Centered Planning (PCP) and Individual Plan of Service (IPOS) training overviews for Clinically Responsible Service Providers in Wayne County.  Please join us for this distance learning opportunity on the ZOOM Platform. 

ABSTRACT:

As Social Workers, we have the responsibility of making sure each person's Individual Plan of Service (IPOS) if specific to their specific preferences, needs, and desires WHILE ALSO making sure we are following good clinical practice standards and the authorization procedures outlined by our various our funders' requirements. This presentation will focus on making sure we maintain solid person-centered and person-directed practices while developing IPOS content and incorporating Golden Thread techniques.  This session is the third in a series of sessions on Person-Centered Planning.  “Getting great services” is obviously not the goal of our field of work -“GETTING A GREAT LIFE” is. This will be an interactive training session that aims to re-frame our views on service provision, with the goal of making our service systems as person-centered and community focused as possible. The presenter will utilize power point slides, short video clips, and active chat engagement with attendees via questions and answers. 
 

COURSE OBJECTIVES:

1. Identify and articulate what is meant by "following the Golden Thread."
 
2. Discuss examples of what clinicians should do if there is a clinical recommendation in an assessment and a person served            does not want to make it a priority in their IPOS.
 
3. Make a compelling case for the need to make planning and service approaches more person-centered through exposure to            the history of the disability rights movement.

TIMED AGENDA:

The trainings will be held on Thursday mornings from 9am to 11am via the ZOOM platform. There will be a single presenter with a power point presentation, short video clips, and active chat engagement with questions and answers. The presenter is skilled and experienced with on-line training methodologies.

  • TRAINING HOURS: These (2) event hours will count toward the annual training hours required by most provider agencies.
